CHRIS Counseling Center
Eligibity Criteria
Georgia children and adolescents between the ages of 6 and 18 with
serious emotional disturbances/addictive disease are eligible for
services at the Counseling Center. (Exceptions to the criteria regarding
age will be determined on a case-by-case basis.)
When an individual is found not eligible for services, the person
is informed as to the reasons, the referral source (if any )is informed
as to the reasons, if consent is obtained, and recommendations are
made for alternative services.

services are offered without discrimination by reason of race, religion,
national origin, gender, sexual orientation or disability.
Admission Process
The CHRIS Counseling Center Intake Team provides customers with
a description of organization services and fees, including a eligibility
requirements and limitations. Customers may need some or all of
the following documents as they go through the admissions process:
- Proof of residency
- Driver's liscence or state ID card
- Proof of income
- Insurance documentation
- Clinical records (for example, IEP's, psychological assessments,
hospital admissions)
An Intake Team member will set up an interview with the youth and
the guardian. This initial interview is usually held at the CHRIS
Counseling Center. At this time the youth, family and/or guardian
may ask questions about the program and facility. A written assessment
will be completed by both the guardian and youth. A liscensed clinician
meets with the youth and guardian for a comprehensive interview
reagrding problems and needs, current family status, social and
medical history, developmental information and other relevant information.
Lastly, initial treatment goals are developed with the input of
client and guardian.
