CHRIS Counseling Center

  • Ages 3 and Above (Including Adults)
  • Community-Based Services (in home, school, work or other community environment)
  • Outpatient Community Clinic
  • Referrals: Self Referrals, Doctors' Offices, Hospitals, Schools, Courts, State Agencies, Other Nonprofit Providers, etc...
  • Payment: The CCC accepts Medicaid, Wellcare, Magellan, Peachcare, Cenpatico, Cigna, TriCare, Blue Cross Blue Shield, Aetna. Our list of insurance providers frequently changes. Please inquire when you book your appointment.

TransitionZ

  • Ages 17-24
  • Mental Health DSM IV Diagnosis by a Licensed Clinician (Assessment provided by CHRIS Counseling Center if needed)
  • Homeless Verification (Eviction notice, emergency shelter paperwork, etc...)
  • No Felony Convictions with regards to drugs, guns, violence, or anything involving minors

CHRIS Choices, A Care Management Entity

  • C-BAY Eligibility: Families with children and youth, ages 5-21, who are either in or at risk to be in a PRTF. The child or youth must have a primary mental health diagnosis and meet Medicaid eligibility.
  • C-BAY Referrals: Must be made through the child/youth's CORE provider or current Psychiatric Residential Treatment Facility.
     
  • Non-Waiver Eligibility:  Families with children and youth, ages 5-21, diagnosed with a persistent mental health disorder, living in the metro-Atlanta areas. Youth with any type of insurance of lack thereof.  This process is at NO cost to the family. 
  • Non-Waiver Referrals: Contact Choices office by phone, email or fax. 
     
  • Youth Matters Eligibility: Families with children and youth located in DeKalb County, ages 5-21, with a primary mental health diagnosis. Eligibility is determined by DeKalb County Department of Family and Children Services or the Department of Juvenile Justice's DeKalb County office.
  • Youth Matters Referrals:  Contact DeKalb County DFCS or Department of Juvenile Justice's DeKalb Multi-Service Center office.
