Our goal is to build a strong community through partnerships and programming that ensures children, youth and families are self-sufficient contributing members of our community. We offer several community-based programs focused on prevention services and empowering individuals and families to reach their full potential. We believe that everyone (caregivers, parents, educators, community partners, children and youth) must become a part of everyday life in the communities where they live, work, study and play.
Choices, A Care Management Entity
- Overview of Choices Choices is one of only four Care Management Entities (CME) in Georgia utilizing the Nationally acclaimed High Fidelity Wraparound model.
- Community Based Alternatives for Youth (CBAY) CBAY is designed to assist families whose children and youth have severe emotional and behavioral challenges through the Wraparound process. The purpose of CBAY is to allow families the choice of intensive community-based care in lieu of residential psychiatric treatment.
- Non-Waiver The Non-Waiver option follows the wraparound process for children, youth, and families who are outside the eligibility requirements for CBAY.
- Youth Matters The ultimate goal of Youth Matters is assisting youth in reaching a goal of permanency and finding a “Forever Family."
TransitionZ
- TransitionZ Program TransitionZ prepares young adults to make a successful transition to become productive, independent, and contributing members of our community.
- CHRIS Clubhouse In partnership with the Georgia Department of Behavioral Health and Developmental Disabilities, we provide young adults ages 16-21 with a place where they can spend their time learning the tools necessary to transition to adulthood successfully while recovering from their mental and/or behavioral disorder.
- LGBTQ Youth We target the needs of homeless and “at risk” Lesbian, Gay, Bisexual, Transgender and Questioning (LGBTQ) youth.
Camp CHRIS
Camp CHRIS is a free, nine-week summer day camp for children and youth ages 5-17 with behavioral or emotional issues that prevent them from attending other “mainstream” summer camps.
