TransitionZ Program

In partnership with the Atlanta Housing Authority, Department of Community Affairs and the United States Department of Housing and Urban Development, we provide young adults ages 17-24 who are homeless, parenting, leaving foster care, juvenile justice and/or mental health systems with supervised apartment living and/or services to obtain or maintain housing. Life coaches guide young people in creating their own futures by using the Transition to Independence Model, a strength-based and youth-driven concept. This model enhances the educational, vocational, housing and community life domains of young people to make a difference in their lives. Life coaches tailor services to meet needs and connect young people to community resources by enhancing competencies. Youth participate in guiding and formulating their individualized service plans. Providing the opportunity to practice daily living skills, TransitionZ prepares young adults to make a successful transition to become productive, independent, and contributing members of our community. Services are outlined by each youth’s individualized service plan.

  • Enhance Competencies to Assist in Self-Sufficiency

Emergency Supportive Services

Emergency support services are offered to assist young adults ages 16-30 in obtaining or maintaining housing.

Services:

  • Service coordination/case management
  • Emergency shelter for young adults ages 16-21
  • If available, financial assistance for rent/mortgage, utilities, vocational training, childcare and transportation.
